FAST FACTS:

• Eligible participants:  Two adults (18 yrs of age or older) insured through Definity Health.
• A family or couple membership can have up to 2 adults, age 18 or older, participating with a maximum reimbursement of $40/month. 
• Members MUST complete the Definity Health enrollment form and provide us with your member ID card, Definity Health insurance card and proof of age.
• Each individual participating in the program must work out 12 times or more per month to earn the reimbursement (credit), up to $20 per month.  Only one workout per day, per member, will be counted towards this program.
• The time period is based on one calendar month; so if you joins on January 15^th,  you would need to reach 12 visits by January 31st and visits would be set back to zero at the start of February.
