Members of fully insured (outside of InstaCare) Blue Cross and Blue Shield of Minnesota groups and individual health plans are eligible for the up to $20-per-month discount program. Members of self-insured groups may also be eligible if their employer has purchased this program.  
How do members know if they’re eligible?

• All eligible members will receive the fitness icon in the upper right hand corner of their medical ID card.

• Members may check the Blue Cross online member service center, “myBlueCross,” at www.bluecrossmn.com to see if the program is listed as part of their benefits.

• Members may also call the customer service number on the back of their member ID card.

Blue Cross Blue Shield of MN:

Eligible members:

          Up to two people per household, 18 years of age or older.

• Fully Insured ~ 12 visits = up to a $20 credit

• Self Insured   ~   8 visits = up to a $20 credit

• CCStpa/Service Co-ops ~ 8 visits = up to a $20 credit
